Care Partners for Seniors
Caring for elderly relatives or friends is a rewarding yet challenging experience. As health care
needs become more complex, caregivers may need assistance in locating and coordinating the
resources to provide support to their loved ones and to help them maintain their independence
as long as possible.
CenVaNet’s Care Partners program provides comprehensive care management to assist the
senior client, their family and/or caregiver with their care needs. The primary goal of Care
Partners is to improve and maintain the health status and quality of life for seniors.
Care Partners assist by providing:
- Local support by RNs and social workers for in and out of town caregivers
- Efficient, expert navigation of the health care system and community resources
- Assessment of client's current status and planning for the future
- Coordination of care, development of individualized care plan and ongoing monitoring
Care Partner's highly experienced staff will meet with the senior client and their caregiver to
perform a comprehensive assessment. The RN and/or social worker will work with the client,
family and physician to develop, implement, monitor and revise an individual care plan.
Services Provided:
- Comprehensive assessment
- Physical health
- Psychosocial and mental health
- Social support and resources
- Home environment and safety (fall risk, fire hazards etc.)
- Functional or self-care ability (dressing, bathing etc.)
- Financial resources (ability to afford medications etc.)
- Long term care needs (advanced directives, nursing care facility)
- Care Plan development in conjunction with physicians and family
- Care Plan management and coordination
- Ongoing monitoring and communication with family and care givers
