Familiar statistics reveal a dramatic increase in frequency and severity of chronic disease and the burden placed on our society. CenVaNet is dedicated to addressing this healthcare crisis by developing better solutions to engage patients and their physicians, to improve the flow of critical health information and to ensure continuity of care across the community.

CenVaNet is one of only fifteen (15) sites selected by the Centers for Medicare and Medicaid Services (CMS) for a pilot project to test whether providing care management services to Medicare fee-for-service beneficiaries with complex chronic conditions can yield better patient outcomes without increasing program costs. The pilot, known as the Medicare Coordinated Care Demonstration (MCCD) utilizes CenVaNet care managers to work closely with seniors and their physicians in central Virginia. CenVaNet’s program is a randomized control trial involving over 1,700 seniors with moderate to severe chronic conditions including cardio-vascular disease, congestive heart failure, diabetes and lung disease.

CenVaNet Registered Nurse and Social Work care managers enhance the patient’s ability to follow their physician’s prescribed treatment plan. Through an optimal balance of home visits and telephonic contact, Care Managers provide care coordination, condition-specific education and ongoing support to patients and their caregivers. Targeted interventions focus on behavioral change and disease-specific self management skills that lead to improved health outcomes.

CenVaNet’s care management programs are supported by a sophisticated and robust care management information system built on the MedVirginia platform that provides outcomes reporting, best practice clinical guidelines, and productivity management tools.
